Registration Instructions for CRA PDI 2014

TWO WAYS TO REGISTER:

US MAIL Download “PDI Registration Form” and send with full payment to:

ONLINE Register online by clicking the “Online PDI Registration” button. ONLINE REGISTRATION AND PAYMENT BY USING PAYPAL or CREDIT CARD ONLY.

GENERAL REGISTRATION INFORMATION

MAIL IN REGISTRATION: Members, Non-members, pre-service teachers and groups may register by downloading the “PDI Registration Form-US Mail”. Once downloaded, completely fill out form and mail back to CRA with full payment. Checks, Credit Cards, District Checks and Purchase Orders are accepted. Be sure to include a phone number and email address where you can be reached in case a question arises. If you wish to attend the CYRM Author’s/President’s/Awards Dinner on Saturday evening, October 18, please indicate, in the appropriate space, the number of tickets you wish to purchase.

If you wish to attend the Author Meet & Greet, Friday evening October 17, please check the appropriate box. This event is included in the price of registration, but is a ticketed event. Space is limited. You Must register to attend. THIS EVENT IS CLOSED. NO TICKETS ARE AVAILABLE.

ONLINE REGISTRATION: PLEASE NOTE THAT IF YOU ARE REGISTERING ONLINE, PAYMENT MUST BE MADE USING PAYPAL OR CREDIT CARD ONLY. Click the “Registration Online” button at the bottom of this page. From the dropdown list, select the appropriate type of Institute Pass and the day(s) you wish to attend. (Member, Non-member, Pre-service Teacher, and Speaker) When complete, click the “View Shopping Cart” button.  Review cart carefully. If any changes need to be made, do so on this page and click update. ie: If ordering extra dinner tickets, click on the quantity box, make change in quantity, then click "update".  If you wish to attend the Author Meet and Greet, Friday evening October 17th, please add this event to your shopping cart from the dropdown list.  This event is included in the price of registration, but is a ticketed event. Space is limited. You MUST register for this event to attend. Continue shopping or check out. At check out, fill in ALL required information on payment page and registration page. When complete, you will receive a receipt.  IF YOU DO NOT COMPLETE PAYMENT USING PAYPAL or CREDIT CARD, YOUR REGISTRATION WILL NOT BE PROCESSED.

NOTE: GROUPS may NOT REGISTER ONLINE. See instructions for Groups, below.

GROUPS: GROUPS may only register by mail or fax. No Group Registrations will be accepted online.

Download registration form from website following MAIL IN REGISTRATION instructions above. Each registrant in group needs to complete a registration form.

If any member of your group wishes to attend the Awards dinner on Saturday evening, October 18, indicate on each individual form, the number of tickets they wish to purchase.

If anyone wishes to attend the Author Meet & Greet, Friday evening October 17, please check the appropriate box on each form. This event is included in the price of registration, but is a ticketed event. Space is limited. You Must register to attend.

PRE-SERVICE TEACHERS: If registering as a pre-service teacher, you MUST submit to CRA,  a copy or proof of class enrollment. If paying by PayPal or Credit Card, please be sure to include with your proof of enrollment, your payment receipt # and date registration was purchased

Mail ALL forms together with payment or copy of Purchase Order. If sending a PO, please include all names of people in your group on the PO. Your district will be invoiced upon receiving all forms with payment or copy of PO. You may also fax your PO and forms to 949-481-8163.

COLLEGE CREDIT Information


Confirmation: All institute registrants will receive a confirmation postcard via US Mail after Sept. 22, 2014. Bring this card to the event. All registration material, including your name badge and any special event tickets purchased may be picked up at On-site Registration. 

If you do not receive a confirmation postcard by Oct. 10, 2014, please call the CRA office at 714-435-1983, to verify your registration has been received and processed.

Registration DEADLINE: All registration forms must be received in the CRA office by October 10, 2014 in order to be processed before the institute begins. After this date, only on-site registrations will be accepted. If registering on-site, please bring your registration form, completed, with you. If paying with a Purchase Order, bring a copy of the PO with you.

Cancellation: All cancellations must be received in writing by Sept. 22, 2014. Cancellations received after Sept. 22 2014 are NON-REFUNDABLE, NO EXCEPTIONS.
